I understand that accepting a Solo or Extra Role in the Spring Ballet requires a strict attendance policy, extra rehearsal commitments, as well as extra financial commitment. By entering my name, I acknowledge that I am aware of and agree to accepting these responsibilities if my dancer is selected for and accepts an additional role.

I understand that after auditions, if my dancer is selected for a Solo or Extra Role, both I and my dancer need to fill out the Agreement and return to Mrs. Katherine by Thursday, August 22nd. If Agreement is not returned by that date, my dancer may risk forfeiting her role.
Rehearsal Time Agreement (enter Parent name)*

"Choreography Days" will be held on Wednesdays 7:15-9:15 in October. Dancers will come to 1 of those days, then shift to Monday Review & Cleaning Sessions. Most rehearsals (after the initial choreography days) will take place on Monday evenings following ballet class 8:00-9:00. If additional one-one-one time with Mrs. Katherine is needed, I have time after 7:15 on Wednesdays.
Audition Agreement & Safety Waiver*

By completing and singing this form, guardian and dancer agree to attend Spring Ballet Audition and remit payment for Audition Fee. Guardian and dancer recognize the risk of personal injury during said audition/course of instruction and will not hold Creative N'rgy DBA Southeast School of Dance or its instructors/staff liable if dancer sustains any injury. Guardian/dancer acknowledge that completing this form and attending audition does not equal a commitment to accept a position as a soloist or understudy. A separate agreement will be sent to select dancers, signed, and turned in to SESD after a Spring Ballet Role acceptance invitation has been sent following auditions.
